Client Agreement
Thank you for choosing Cedar Grove Veterinary Clinic. To help us provide the best care for you and your pet(s), we ask that you review this information, then fill out and sign the agreement form. This information outlines our clinic policies related to your relationship with our team, care availability, payments, appointments, and media permissions.

Veterinarian-Client-Patient Relationship (VCPR)
We follow state and national standards for the veterinarian-client-patient relationship. This means:
We must see your pet in person at least annually for exams and follow-ups to maintain a valid VCPR.
Without a current VCPR, we cannot legally prescribe medications or offer specific medical advice.
Records of all visits, diagnostics, and treatments will be kept by our staff.
Emergency & Urgent Care
While we’re not a 24-hour emergency facility, we do provide urgent and emergency services based on availability.
If we are at capacity, we may need to refer you to another clinic or to Blue Pearl Pet Hospital - Duluth, depending on the severity of the emergency.
A $157 fee will be charged to your account if you do not show up for an emergency appointment after it has been scheduled.
To help us provide ongoing care, please notify us of any outside visits, as medical records may be delayed.
Payment Policy
Payment is due at the time of service.
We accept cash, checks, major credit cards, and Care Credit.
A 3% processing fee applies to all card payments.
Returned checks are subject to a $30 fee and may limit future payment options.
Unpaid balances may lead to account suspension or referral to collections.
Late Arrivals & Missed Appointments
Your scheduled time is important to us and to other clients. To help us stay on track:
Missed Appointments: A $67 fee applies if you miss an appointment without notice. After a second missed visit, a deposit and any outstanding balance must be paid before rescheduling. After three missed visits, care may be discontinued.
Surgical Appointments: Cancellations with less than 3 business days’ notice or no-shows are subject to a $157 fee.
Late Arrivals: If you are 5 -10 minutes late, we MAY need to reschedule, admit your pet for a “drop-off” (which requires you to return at a designated time for your pet), or adjust your appointment based on the day’s schedule.
We understand that emergencies happen - please let us know as soon as possible if you’re running late or need to cancel.
Media Release
We occasionally take photos or videos of our patients for use in educational or promotional materials, including social media. In the following form, you will be asked to indicate your preference in this regard.
